Sears model 200 pump shotgun, 12 gauge, 2 3/4 chamber Dash modified. Model number 273.2250 Sears Model 200 is essentially an alternate Winchester 1200.  It was produced by Winchester to be sold in Sears, Roebuck and company stores as their store-brand pump shotgun.  This model was introduced in 1964 and made for only one year.
